Required Forms and Information Homeowners Are to Provide to Lender
- 2012 RMA Request for Mortgage Assistance
- Financial Statement and Profit & Loss Statement
- Signed IRS Form 4506T (Request for Transcript of Tax Return)
- Hardship letter which briefly states Homeowner’s current situation and reason for delinquency.
- Last 30 days of paystubs for those listed on the mortgage (borrower).
- Proof of any other income: SSI, unemployment, pension etc.
Also include income for anyone living in the home who contributes to household expenses who wishes to be included (non-borrower)
If receiving rental income, provide a copy of the lease and bank statements showing deposits of the rent checks. - For alimony, child support or maintenance received by Homeowner, provide a copy of the divorce decree or any other written legal agreement and bank statements proving receipt of the income. (Alimony, child support or separate maintenance income need not be disclosed if you do not choose to have it considered for repaying your mortgage debt.)
- Last two months of bank statements for all bank accounts, all pages even blank pages. Please note that a transaction history is not acceptable.
- Most recent 2 years of Federal tax returns, must be signed and dated even if filed electronically. Include all schedules.
- Most recent mortgage statement.
- Mortgage statements for 2nd or 3rd mortgage and Home Equity Line of Credit, if applicable.
- A copy of Neighborhood Association dues invoice and copy of check paying dues. Or a letter stating no association dues are owed.
